fa یافته‌های تصویربرداری بیماران مبتلا به آنفلوانزا A مراجعه‌کننده به مرکز درمانی قائم شهر Imaging Findings in Patients With Influenza A Infection Referring To Razi Hospital عفوني infection گزارش کوتاه Brief Report سابقه و هدف: آنفولانزا همچنان یکی از نگرانی‌های مهم بهداشت جهانی است و تحقیقات مستمر برای درک تظاهرات بالینی و شاخص‌های پیش‌آگهی آن را تضمین می‌کند. تشخیص دقیق و استاندارد طلایی علت پنومونی مبتنی بر آزمایش‌های تشخیصی میکروبیولوژیکی است، با این حال، داده‌های بالینی و رادیولوژیکی می‌توانند در محدود کردن تشخیص افتراقی مفید باشند. این مطالعه با هدف ارزیابی جامع یافته‌های رادیولوژی در بیماران مبتلا به آنفولانزا در مرکز آموزشی درمانی رازی قائم‌شهر انجام شد. علاوه بر این، این تحقیق با هدف تعیین هر گونه همبستگی بالقوه بین این تظاهرات رادیولوژیکی و ویژگی‌های جمعیتی و هم‌چنین بالینی مختلف انجام شد. مواد و روش‌ها: در این مطالعه کوهورت آینده نگر، بیمارانی که با علائم آنفلوانزا به بیمارستان رازی قائم‌شهر در بازه زمانی سال ۹۸-۱۳۹۷ بستری شدند، مورد ارزیابی قرار گرفتند. بررسی رادیولوژی طی ۲۴ ساعت از ورود به بیمارستان انجام شد. تجزیه و تحلیل آماری توسط نرم‌افزار SPSS با در نظر گرفتن 0/05P< انجام شد. یافته‌ها: از بین این افراد، ۳۰ نفر مرد بودند که 57/7 درصد از گروه را تشکیل می‌دادند، در حالی که ۲۲ بیمار باقیمانده زن بودند که ۳/۴۲ درصد از جمعیت مورد مطالعه را تشکیل می‌دادند. یکی از نگرانی‌های جدی، زیر گروهی متشکل از ۹ بیمار (17/3 درصد) به دلیل شدت وضعیت خود نیاز به بستری در بخش مراقبت‌های ویژه (ICU) داشتند. متأسفانه، در این زیر گروه، ۷ بیمار (77/8 درصد) در نهایت به عفونت آنفولانزا تسلیم شدند، که بر شدت این بیماری ویروسی تأکید می‌کند. با بررسی ارزیابی‌های رادیولوژیک، این مطالعه نشان داد که درحالی‌که ۱۳ بیمار (۲۵ درصد) یافته‌های طبیعی را در مطالعات تصویربرداری نشان دادند، اکثریت قابل ملاحظه ای از ۳۹ بیمار (۷۵ درصد) درجات مختلفی از ناهنجاری‌های ریوی را نشان دادند. این ناهنجاری‌ها با طیفی از تظاهرات رادیولوژیکی مشخص می‌شوند که نفوذ آلوئولی، کدورت‌های شیشه‌ای و ادغام به‌عنوان شایع‌ترین یافته‌ها ظاهر می‌شوند. به‌طور خاص، ارتشاح آلوئولی، که در ۱۴ بیمار (26/9 درصد) مشاهده شد، به‌عنوان یک شاخص آشکار در ارتباط با پیش‌آگهی ضعیف در میان گروه‌های آلوده به آنفولانزا ظاهر شد. استنتاج: یافته‌های این تحقیق بر ارتباط معنی‌داری بین وجود ناهنجاری در تصویر برداری ریه به خصوص یافته انفیلتراسیون الوئولار با پیش آگهی نامناسب در بیماران مبتلا به آنفلوانزا تاکید می‌کند. این الگوهای رادیولوژیکی به عنوان نشانه‌گرهای پیش‌آگهی ارزشمندی هستند که بینش‌های مهمی را درمورد مسیر بالینی بیماری ارائه می‌کنند و به‌طور بالقوه به تدوین مداخلات استراتژیکی کمک می‌کنند. در نتیجه، این مطالعه نقش محوری ارزیابی‌های رادیولوژیکی، به‌ویژه شناسایی ارتشاح آلوئولی، در پیش‌بینی نتایج در میان افراد مبتلا به آنفولانزا را برجسته می‌کند. این یافته‌ها بر ارتباط استفاده از ارزیابی‌های رادیولوژیکی به‌عنوان ابزار ضروری در تصمیم‌گیری بالینی تأکید می‌کنند، بنابراین مداخلات به موقع و افزایش مراقبت از بیمار در پروتکل‌های مدیریت آنفولانزا را تسهیل می‌کنند. Background and Purpose: Influenza remains a significant global health concern, necessitating continuous research to understand its clinical manifestations and prognostic indicators. While the accurate diagnosis of pneumonia etiology is based on microbiological tests, clinical and radiological data can help narrow the differential diagnosis. This study, conducted at the Razi Educational and Medical Center in Ghaemshahr, aimed to comprehensively evaluate the radiological findings among patients diagnosed with influenza. Additionally, it sought to explore potential correlations between these radiological manifestations and various demographic and clinical characteristics. Materials and Methods: Using a prospective cohort design, the study enrolled individuals who were admitted to Razi Hospital in Ghaemshahr with symptoms indicative of influenza from 2018 to 2019. Upon admission, all patients underwent comprehensive CT scans within 24 hours to assess the extent and nature of pulmonary involvement. The data collected from these evaluations were analyzed statistically using SPSS software, with a significance level set at P<0.05. Results: The cohort consisted of 52 patients diagnosed with influenza, confirmed through both clinical assessments and laboratory tests. Among them, 30 were male (57.7%) and 22 were female (42.3%). Notably, 9 patients (17.3%) required admission to the intensive care unit (ICU) due to the severity of their condition. Of these ICU patients, 7 (77.8%) unfortunately succumbed to the influenza infection, highlighting the seriousness of this viral illness. Regarding the radiological assessments, the study found that while 13 patients (25%) showed normal findings on imaging studies, a significant majority of 39 patients (75%) exhibited varying degrees of pulmonary abnormalities. These abnormalities were characterized by a range of radiological manifestations, with alveolar infiltration, ground-glass opacities, and consolidation being the most common findings. Alveolar infiltration, observed in 14 patients (26.9%), emerged as a notable indicator of poorer prognosis among the influenza-infected cohort. The findings of this study underscore a significant correlation between abnormal lung imaging—particularly alveolar infiltration—and an unfavorable prognosis in patients with influenza. These radiological patterns serve as valuable prognostic markers, providing critical insights into the clinical course of the illness and potentially aiding in the formulation of strategic interventions. Conclusion: The study highlights the pivotal role of radiological assessments, particularly the identification of alveolar infiltration, in predicting outcomes among individuals with influenza.
