fa بررسی اثربخشی ترکیب سنتزی ایندول هیدرازنوتیازولیدینون بر روی ایزوله‌های کاندیداهای جدا شده از عفونت‌های دهانی Evaluation of the Efficacy of Synthetic Indole-Hydrazono-Thiazolidinone Compounds on Candida Isolates from Oral Infections دندانپزشکی Dentistry پژوهشي-کامل Research(Original) سابقه و هدف: کاندیدیازیس دهانی از شایع‌ترین عفونت‌های سطحی و جلدی مخاطی می‌باشد که مخاط دهان را درگیر می‌کند. این بیماری توسط گونه‌های مختلف از جمله کاندیدا آلبیکنس در دهان ایجاد می‌شود. امروزه درمان با دارو های ضد قارچ گروه پلی ان و آزول، سلول های بیمار را تحت تاثیر قرار می‌دهند. وجود محدودیت‌هایی در درمان بیماری‌های قارچی مانند کمبود و گرانی داروهای ضدقارچی، عوارض جانبی آن‌ها و نیز گسترش مقاومت‌های دارویی آزول در میان گونه‌های کاندیدا به عنوان یک مشکل مهم می‌باشد. با توجه به مشکل درمان‌های حاضر، توجه پژوهشگران به داروهای ضد قارچی جدید به‌ویژه ترکیبات هتروسیکلیک جلب شده است. از ترکیبات هتروسیکلیک به‌طور وسیع در صنایع داروسازی استفاده می‌شود. مشتقات تیازولیدینون، مقاومت قارچی و باکتریایی که توسط تعداد زیادی از آنتی‌بیوتیک و داروها ایجاد می‌شود را تضعیف نموده و همچنین اکسیندول ها فعالیت دارویی و بیولوژیکی قابل توجهی از خود نشان می‌دهند. با توجه به نکات ذکر شده در بالا و در نظر گرفتن خصوصیات بیولوژیکی قابل توجه تیازولیدینون‌ها و اکسیندول، استفاده از آن‌ها برای تولید داروی ضد قارچی جدید مورد توجه محققان و پژوهشگران است. لذا این تحقیق به منظور بررسی اثر بخشی ترکیب سنتزی ایندول هیدرازونو تیازولیدینون بر روی ایزوله های قارچی کاندیدای جدا شده از عفونت دهانی انجام می شود. مواد و روش‌ها: حساسیت دارویی 100 ایزوله بالینی کاندیدا شامل 53 ایزوله کاندیدا آلبیکنس، 22 ایزوله کاندیدا تروپیکالیس، 17 ایزوله کاندیدا پاراپسیلوزیس و 8 ایزوله کاندیدا گلابراتا نسبت به مشتقات 4a و 4b ایندول هیدرازنو تیازولیدینون و داروی نیستاتین به روش میکرودایلوشن براث و براساس دستورالعمل CLSI-M27S4 برای تعیین حساسیت دارویی و مخمرها و برای شناسایی گونه‌ها براساس روش فنوتیپی کروم آگار و ژنتیکی PCR-RFLP استفاده گردید. دامنه رقت دارویی ترکیبات و داروهای ضدقارچی به ترتیب 16-0/016 میکروگرم/میلی‌لیتر بوده است. غلظتی از ترکیبات که حداقل 50 درصد مهار رشد نسبت به گروه کنترل مثبت را داشتند، به‌عنوان MIC (حداقل غلظت مهاری رشد) در نظرگرفته شد. آنالیز آماری با نرم‌افزار 20 SPSS انجام و سطح معنی‌داری به‌صورت 0/05>P در نظرگرفته شد. یافته‌ها: نتایج نشان داد بین MIC مشتقات 4a و 4b ایندول هیدرازنوتیازولیدینون و داروی نیستاتین علیه گونه‌های کاندیدا آلبیکنس، کاندیدا تروپیکالیس و کاندیدا پاراپسیلوزیس اختلاف معناداری وجود داشت و بین میانگین MIC های مشتقات با یکدیگر نیز اختلاف معنی‌داری مشاهده گردید؛ اما بین MIC این مشتقات با داروی نیستاتین علیه کاندیدا گلابراتا اختلاف معناداری وجود نداشت. استنتاج: تاثیرگذاری مشتقات ایندول هیدرازونوتیازولیدینون در مقایسه با داروی نیستاتین برای ایزوله‌های کاندیدا آلبیکنسس، کاندیدا تروپیکالیس و کاندیدا پاراپسیلوزیس کم‌تر بود، در حالی که تاثیرگذاری آن‌ها بر روی گونه‌های کاندیدا گلابراتا نسبت به نیستاتین در یک محدوده بود. با بررسی اثرات سایتوتوکسیسیتی و عوارض جانبی این مشتقات و هم‌چنین انجام بررسی‌های بیش‌تر برروی آن‌ها می توان در آینده از این ترکیبات برای ساخت و سنتز داروهای ضدقارچی جدید و آلترناتیوهای درمانی بهره برد.   Background and purpose: Oral candidiasis is one of the most common infections affecting the mucosal surfaces and skin, specifically involving the oral mucosa. This disease is primarily caused by various species of Candida, including Candida albicans. Current treatments with polyene and azole antifungal drugs can affect patient cells, and there are also several limitations in treating fungal infections. These limitations include the shortage and high cost of antifungal drugs, their side effects, and the increasing resistance to azole drugs among Candida species. Given these challenges, researchers are exploring new antifungal drugs, particularly heterocyclic compounds. Heterocyclic compounds are widely used in the pharmaceutical industry. Thiazolidinone derivatives can weaken the fungal and bacterial resistance caused by many antibiotics and drugs. Additionally, oxindoles show significant pharmacological and biological activity. Given these factors and the notable biological properties of thiazolidinones and oxindoles, their potential for developing new antifungal drugs has gained interest among researchers. Therefore, this study aims to investigate the effectiveness of synthetic indole-hydrazono-thiazolidinones on Candida isolates obtained from oral infections. Materials and methods: Antifungal susceptibility testing was conducted on 100 clinical isolates of Candida, including 53 Candida albicans isolates, 22 Candida tropicalis isolates, 17 Candida parapsilosis isolates, and 8 Candida glabrata isolates. The susceptibility of these isolates to 4a and 4b indole-hydrazono-thiazolidinone derivatives and nystatin was assessed using the broth microdilution method, based on CLSI-M27S4 guidelines for determining drug sensitivity in yeasts. Species identification was performed using phenotypic chrome agar and genetic PCR-RFLP methods. The compounds and antifungal drugs were tested at a dilution range of 0.016–16 μg/ml. The minimum inhibitory concentration (MIC) was defined as the concentration that resulted in at least 50% inhibition of growth compared to the positive control group. Data were analyzed using SPSS version 20 software, with a significance level of P<0.05. Results: The results showed a significant difference in MIC values between the indole-hydrazono-thiazolidinone derivatives 4a and 4b, and nystatin against Candida albicans, Candida tropicalis, and Candida parapsilosis. There was also a significant difference in the average MIC values of the derivatives. However, no significant difference was observed in MIC values between these derivatives and nystatin against Candida glabrata. Conclusion: The antifungal activity of the indole-hydrazono-thiazolidinone derivatives was lower than that of nystatin against Candida albicans, Candida tropicalis, and Candida parapsilosis. However, the activity against Candida glabrata was comparable to that of nystatin.
