fa بررسی شیوع پوسیدگی اکلوزال و فاکتورهای کلینیکی مرتبط با آن در مولرهای اول دائمی نیمه ‌رویش ‌یافته در کودکان 6 و 7 ساله شهر ساری در سال 1304 – ۱۴۰۳ Investigation of the Prevalence of Occlusal Caries and Related Clinical Factors in Partially Erupted First Permanent Molars Among 6- and 7-Year-Old Children in Sari, Iran, 2024-2025 دندانپزشکی Dentistry پژوهشي-کامل Research(Original) سابقه و هدف: دندان مولر اول دائمی به‌عنوان نخستین دندان دائمی رویش‌ یافته، به دلیل تماس ناقص با دندان مقابل، آناتومی پیچیده و دشواری رعایت بهداشت دهان، مستعد بروز پوسیدگی اکلوزال است. از این رو، مطالعه حاضر با هدف تعیین شیوع پوسیدگی اکلوزال در مولر اول دائمی نیمه‌ رویش ‌یافته در کودکان ۶ و ۷ ساله شهر ساری، انجام پذیرفت. مواد و روش ‌ها: این مطالعه به ‌صورت توصیفی– تحلیلی و مقطعی، در سال 1404-1403 بر روی ۳۲۹ کودک ۶ تا ۷ ساله دارای حداقل یک مولر اول دائمی نیمه ‌رویش ‌یافته انجام شد. نمونه‌ها با روش خوشه‌ای و در دسترس از مدارس ابتدایی شهر ساری انتخاب شدند. پس از اخذ رضایت‌نامه آگاهانه والدین، معاینات کلینیکی توسط دانشجوی آموزش ‌دیده دندانپزشکی و با استفاده از آینه و سوند در نور مناسب انجام شد. پوسیدگی اکلوزال بر اساس معیار (ICDAS) international caries detection and assessment system، وضعیت بهداشت دهان با شاخص (S-DI) Simplified Debris و کمیت پوسیدگی در دندان ها با شاخص‌های dmft/DMFT ثبت گردید. داده‌ها با استفاده از نرم‌افزار SPSS نسخه ۲۷ و در سطح معنی‌داری 0/05 تحلیل شدند. یافته‌ها: شیوع پوسیدگی اکلوزال در مولر اول دائمی نیمه‌ رویش ‌یافته 1/40 درصد بود. پوسیدگی به‌طور معنی‌داری در دختران، کودکان ۷ ساله، دانش‌آموزان مدارس دولتی و مدارس ناحیه ۱ بیش‌تر مشاهده شد. هم‌چنین افزایش شاخص dmft و وضعیت نامطلوب بهداشت دهان (S-DI بالاتر) با افزایش شانس بروز پوسیدگی ارتباط مستقیم و معنی‌داری داشت (0/05< P). استنتاج: پوسیدگی اکلوزال در مولر اول دائمی نیمه رویش یافته در کودکان شهر ساری شیوع بالایی دارد و عوامل متعددی از جمله جنسیت، سن، نوع مدرسه، و وضعیت بهداشت دهان و دندان در این پوسیدگی نقش دارند. یافته‌ها بر اهمیت اجرای برنامه‌های پیشگیرانه، به‌ویژه در مدارس دولتی و مناطق محروم، تأکید می‌کنند. Background and purpose: The permanent first molar, as the first permanent tooth to erupt, is highly susceptible to occlusal caries due to incomplete contact with the opposing tooth, complex anatomy, and difficulty in maintaining oral hygiene. Therefore, the present study aimed to determine the prevalence of occlusal caries in partially erupted permanent first molars among 6- and 7-year-old children in Sari, Iran. Materials and methods: This descriptive-analytical cross-sectional study was conducted during 2024-2025 among 329 children aged 6-7 years who had at least one partially erupted permanent first molar. Participants were selected from primary schools in Sari using cluster and convenience sampling methods. After obtaining written informed consent from the parents, clinical examinations were performed by a trained dental student using a dental mirror and probe under adequate lighting conditions. Occlusal caries was assessed according to the International Caries Detection and Assessment System (ICDAS). Oral hygiene status was evaluated using the Simplified Debris Index (S-DI), and dental caries experience was recorded using the dmft/DMFT indices. The data were analysed using SPSS software, version 27, at a significance level of 0.05. Results: The prevalence of occlusal caries in partially erupted permanent first molars was 40.1%. Caries was significantly more prevalent among girls, 7-year-old children, students attending public schools, and students enrolled in schools located in District 1. Furthermore, higher dmft scores and poorer oral hygiene status, indicated by higher S-DI scores, were significantly associated with higher odds of occlusal caries (P < 0.05). Conclusion: This study demonstrated a high prevalence of occlusal caries in partially erupted permanent first molars among children in Sari. Several factors, including sex, age, school type, and oral hygiene status, were associated with the occurrence of caries.
