شیوع اختلالات عادت ماهانه و عوامل مرتبط با آن در دختران دبیرستانی شهرستان ساری

سابقه و هدف: اختلالات عادت ماهانه شامل دیسمنوره، آمنوره و اولیگومنوره همواره از شکایات آزاردهنده زنان، به خصوص در دوره جوانی، میباشند که باعث کاهش کیفیت زندگی این بیماران میشود. این مطالعه با بررسی دختران دبیرستانی شهرستان ساری به بررسی شیوع این اختلالات و عوامل مرتبط با آنها پرداخت.
مواد و روشها: با طراحی یک مطالعه توصیفی و تحلیلی به صورت مقطعی نمونهگیری به صورت خوشهای انجام شد و در مجموع 1140 دختر دبیرستانی وارد مطالعه شدند. دختران مورد مطالعه به یک پرسشنامه با 24 سؤال پاسخ دادند. قد، وزن، دور کمر و دور باسن دانشآموزان اندازهگیری شد. در انتها دادههای جمعآوریشده توسط نرمافزار SPSS نسخه 17 و با شاخصهای مرکزی و پراکندگی و آزمونهای 2χ، Student-t و Mann Whitney تجزیه و تحلیل شدند.
یافتهها: میانگین سن نمونهها 2/2 ± 4/15 سال و میانگین شاخص توده بدنی (Body mass index یا BMI) آنها 1/6 ± 4/21 کیلوگرم بر متر مربع بود. متوسط تعداد روزهای عادت ماهانه در هر دوره 1/1 ± 3/6 روز بود. 2/1 درصد نمونهها آمنوره، 6/1 درصد منوراژی، 2/32 درصد لکهبینی بین عادت ماهانه، 5/5 درصد پلیمنوره، 9/9 درصد اولیگومنوره، 5/95 درصد حداقل یکی از شکایات مربوط به دیسمنوره و 8/66 درصد یکی از علایم سندرم قبل از قاعدگی را داشتند. خونریزی طولانیتر از 10 روز فقط در 2 درصد دختران وجود داشت ولی خونریزی 8 روز و بیشتر در 10 درصد نمونهها دیده شد. 4/32 درصد از نمونهها به دلیل خونریزی طولانی و 2/28 درصد به دلیل اولیگومنوره به پزشک مراجعه کرده بودند. در کسانی که پلیمنوره داشتند مصرف هورمون به طور معنیداری بیش از افرادی بود که این مشکل را نداشتند (15 درصد در مقابل 7/3 درصد).
استنتاج: این مطالعه نشان داد که میان متغیرهای مورد بررسی در این تحقیق با سایر مطالعات تفاوتهایی وجود دارد که علل آن را میتوان تفاوتهای فرهنگی و نژادی دانست.
Background and purpose: Menstrual disorders such as dysmenorrhea (painful period), amenorrhea (no menstruation), oligomenorrhea (prolonged menstruation) and irregulars menstruations which are always the painful complaint of women, especially in young women, that reduces their quality of life. In this investigation, the prevalence of menstrual disorders and its associated factors were examined in high school girls in Sari during 2010 to 2011.
Materials and methods: In this cross-sectional, descriptive and analytic study, sample size was estimated by formula and 1140 high school girls were questioned and answered to 24 questions according to the questionnaire. Height, weight and waist and hip circumference were measured. The results were analyzed chi square, t-test, and Mann-Whitney test.
Results: The mean age of the subjects were 15.4 ± 2.2 years and their body mass index (BMI) was 21.4 ± 6.1 kg/m2. The mean days of menstrual period were 6.3 ± 1.1 days. Amenorrhea was observed in 1.2%, menorrhagia in 1.6%, spotting in 32.2%, polymenorrhea in 5.5%, oligomenorrhea in 9.9%, dysmenorrhea in 95.5 % and at least one sign of premenstrual syndrome in 66.8% of them. 12% of the subjects had prolonged bleeding (2% more than 8 days and 10% more than 8 days). Referring to physician was seen in 32.4% because of the prolonged bleeding and because of oligomenorrhea in 28.2%. Using the hormone was significantly high in girls who had polymenorrhea (15% vs. 3.7%).
Conclusion: This study was different from the results of previous studies which were performed in other countries that can be due to cultural and ethnic differences.
