بررسی میزان متابولیت های ادراری ترکیبات آلی فرار و عوامل مرتبط با آن در کارگران شاغل در صنعت پتروشیمی در سال1391

سابقه و هدف: پایش بیولوژیکی برای کارگران در معرض حلالهای شیمیایی، مکمل پایش محیطی میباشد و میتواند در ارزیابی ریسک بهداشتی مواجهه با ترکیبات سمی مفید و کارآمد واقع شود. هدف از این مطالعه پایش محیطی و بیولوژیکی مواجهه کارگران با ترکیبات آلی فرار در صنعت پتروشیمی می باشد.
مواد و روش ها: این مطالعه از نوع توصیفی و به روش مقطعی میباشد. نمونههای هواو ادرار از 104 کارگرشاغل در مجتمعهای پتروشیمی جمعآوری گردید. به منظور تعیین غلظت ترکیبات بنزن، تولوئن، ایزومرهای گزیلن، فنول و متیل اتیل کتون، در هوای استنشاقی افرادبر اساس روش 2549NIOSHاز هوای تنفسی آنها نمونهبرداری و تجزیه شد. به منظور تعیین غلظت متابولیت ادراری ترکیبات مورد بررسینمونههای ادرار با استفاده از دستگاههای گاز کروماتوگرافی مایع با عملکرد بالا و گاز کروماتوگرافی تجزیه شدند. مقدار کراتینین ادراری توسط دستگاه اسپکتروفتومتر UV-VIS اندازهگیری شد. در این پژوهش از نرم افزار آماری SPSS16 و آزمون های t گرسیون خطی استفاده گردید.
یافته ها: میانگین غلظت بنزن، تولوئن، ارتو و متا گزیلن، پارا گزیلن، فنول و متیل اتیل کتون هوای تنفسی به ترتیب برابر با192/1،300/1، 97/1، 290/2، 0116/0و 243/2پی پی ام بود. میانگین غلظت ترانس ترانس موکونیک اسید، هیپوریک اسید، 2و3 متیل هیپوریک اسید، 4متیل هیپوریک اسید و فنول ادراری به ترتیب 1431 میکروگرم بر گرم کراتینین، 3949/0، 4444/0، 1483/0 گرم بر گرم کراتینین و 5850/0میلیگرم بر گرم کراتینین و غلظت متیل اتیل کتون ادراری برابر با 154/0 میلی گرم بر لیتر بود. بین غلظت ترکیبات آلی فرار در هوا و متابولیت ادراری آنها ارتباط خطی و مستقیم وجود داشت (05/0>P).نتایج نشان داد که با افزایش ساعت کار هفتگی، میانگین غلظت دفع ادراری ترانس، ترانس- موکونیک اسید، ایزومرهای متیل هیپوریک اسید و فنول افزایش مییابد(05/0>P).
استنتاج: میانگین غلظت بنزن هوای تنفسی، ترانس و ترانس- موکونیک اسید ادراری از استاندارد تعیین شده توسط مرکز سلامت محیط و کار کشورو مجمع دولتی متخصصین بهداشت صنعتی آمریکا بیشتر بود و غلظت دیگر ترکیبات مورد بررسی کمتر ازحدود استاندارد تعیین شده توسط همین سازمانها بود. نتایج حاکی از این است که مصرف سیگار موجب افزایش میزان دفع ادراری تمامی متابولیتها به جز هیپوریک اسید میشود.
Background and purpose: Biological monitoring for workers exposed to solvents includes complementary environmental monitoring which is useful in toxic chemical risk assessments. The aim of this study was environmental and biological monitoring of workers exposed to volatile organic compounds (VOCs) in petrochemical industry.
Material and Methods: Air and urine samples were collected from 104 individuals working in petrochemical industry. NIOSH 2549 method was used to determine the concentrations of benzene, toluene, xylene isomers, phenol, and methyl ethyl ketone in breathing environment. In order to determine the concentration of urinary metabolites, urine samples were analyzed by high performance liquid chromatography and gas chromatography. Urinary creatinine level was measured by UV-VIS spectrophotometer. To analyze the data t-test and linear regression were performed in SPSS V.16.
Results: The mean concentrations of benzene, toluene, O and m-xylene, P-xylene, phenol and methyl ethyl ketone in breath samples were 1.192, 1.300, 1.97, 1.290, 0.0116, and 2.243 ppm, respectively. The mean concentrations of urinary trans, t- MA, hippuric acid, 2, 3 and 4-methyl hippuric acid, and phenol were 1431 µgr/grCr, 0.3949, 0.4444, 0.1483 gr/grCr, and 0.5850mgr/grCr creatinine, respectively. Concentrations of urinary methyl ethyl ketone were 0.154 mgr/lit. There was a direct linear relationship between concentration of VOCs in the air and urinary metabolites (P>0.05). The results showed that increase in working hours per week resulted in increased level of urinary t, t- MA, methyl hippuric acid isomers and phenol (P>0.05).
Conclusion: The mean concentration of benzene in breathing environment, urinary t, and t- MA was higher than the ACGIH's Threshold Limit Values (TLV) and Biological Exposure Indices (BEI) while the mean concentrations of other compounds were lower than TLV and BEI. We also found that smoking increased urinary excretion of all metabolites expect hippuric acid.
