بررسی ضرورت رادیوگرافی در دررفتگی قدامی مفصل شانه

<span style="font-weight: bold">سابقه و هدف:</span>دررفتگی قدامی شانه، شایع ترین دررفتگی مفصلی در بین مفاصل بزرگ می باشد. در اکثر موارد، این دررفتگی در بخش اورژانس جااندازی می گردد. به طور متداول قبل و بعد از جااندازی، پرتونگاری شانه درخواست می شود. هدف از این مطالعه، بررسی ضرورت انجام پرتونگاری شانه در درمان بیماران مشکوک به دررفتگی شانه در بخش اورژانس بود.<br><span style="font-weight: bold">مواد و روش ها:</span>این مطالعه Case series در طول یک سال در بخش اورژانس بیمارستان های رسول اکرم و شهدای هفتــم تیر تهران بر روی 116 بیمار مشـکوک به دررفتگی قدامـی شانه انجام گرفت. در ابتدا و انتهای جااندازی و قبل از انجام پرتونگاری، احتمال دررفتگی یا جااندازی کامل توسط پزشکان اورژانس ارزیابی و ثبت گردید. در نهایت پرتونگاری قبل و بعد از جااندازی، جهت تعیین دقت تشخیصی پزشکان توسط یک ارتوپد، بازبینی و نتایج مقایسه گردید.<br><span style="font-weight: bold">یافته ها:</span>84 نفر (72 درصد) از افراد مورد مطالعه، مرد و 32 نفر (28 درصد) زن بودند. میانگین سنی افراد 31.5±8.5 بود. 30 نفر دررفتگی تکرار شده شانه با مکانیسم غیر ضربه (Trauma) داشتند (گروه 1) و 86 نفر، اولین مورد دررفتگی شانه یا دررفتگی با مکانیسم ضربه داشتند (گروه 2). دقت تشخیصی پزشک در تعیین دررفتگی در گروه 1، معادل 100 درصد و در گروه 2 معادل 98 درصد بود. تشخیص اشتباه جااندازی کامل، فقط در موارد همراه با شکستگی دیده شد. از نظر دقت تشخیصی در زمان اطمینان پزشک از وجود دررفتگی یا جااندازی درست، اختلاف معنی داری بین قضاوت بالینی پزشکان اورژانس و ارتوپد دیده نشد.<br><span style="font-weight: bold">استنتاج:</span> مطالعه حاضر نشان داد که قضاوت بالینی پزشک با کمک معاینه فیزیکی مفصل شانه در تعیین وجود دررفتگی قدامی شانه و همچنین جااندازی کامل آن بسیار دقیق و قابل اطمینان است. پرتونگاری اولیه باید در موارد دررفتگی به علت ضربه گرفته شود. پرتونگاری جهت تایید محل صحیح مفصل جاانداخته شده باید در دررفتگی های همراه با شکستگی و یا در زمان شک پزشک انجام گیرد.

<span style="font-weight: bold">Background and Purpose: </span>Anterior shoulder dislocation is the most common major joint dislocation. In most cases, this dislocation is being relocated in emergency departments. Routinely, pre and post reduction radiographs are performed. This study was done to determine the necessity of radiographs in the emergency department for management of patients with suspected anterior shoulder dislocation.

<span style="font-weight: bold">Materials and Methods:</span> In this case series study, 116 patients suspected of anterior shoulder dislocation were referred to Hazrat Rasoul Akram and Haftome Tir Hospitals emergency departments in Tehran, and were investigated for a one year period. The emergency physicians evaluated and documented the possibility of dislocation or relocation, before obtaining radiographs. Outcome measures were the assessment of joint positions on the x-rays by an orthopedic surgeon.
<br><span style="font-weight: bold">Results: </span>84 cases (72%) were male and 32 cases (28%) were female. Mean age of the patients was 31.5±8.5 yrs. Thirty (30) patients had recurrent dislocations without traumatic mechanism (group 1) and eighty six (86) patients had no prior dislocation or a blunt mechanism of injury (group 2). The accuracy of the emergency physician’s assessment in dislocations was 100% in group 1 and 98% in group 2. False assessments occurred only in patients with fractures. There is no significant difference between emergency physicians and orthopedic surgeons in the assessment of dislocations and relocations, when the emergency physicians did accuralty diagnose them.
<br><span style="font-weight: bold">Conclusion:</span> Our study showed that the physicians are highly accurate in clinical determination of anterior shoulder dislocation and relocation. Pre-reduction films should be obtained when the mechanism of injury is trauma. Post-reduction films should be obtained in fracture-dislocations or when the physicians are uncertain of correct relocation.
