سابقه و هدف: بیشترین درد تجربه شده در صدمات سوختگی طی تعویض پانسمانها اتفاق میافتد. دردی که نیازمند مقادیر بالایی از داروهای مخدر است. شواهدی وجود دارد، مبنی بر اینکه تغییرات در تحریکپذیری قشر مغز - توسط تحریک کاتدی قشر حسی پیکری- در تسکین درد حاد مؤثر است. لذا این مطالعه با هدف تعیین تأثیر تحریک الکتریکی مغز با جریان مستقیم (tDCS) بر کیفیت درد پانسمان سوختگی انجام شد.
مواد و روشها: برای انجام این مطالعه کار آزمایی بالینی تصادفی دو گروهی، 60 بیمار واجد شرایط بستری در بخش سوختگی بیمارستان امام رضا (ع) مشهد بهصورت نمونهگیری در دسترس وارد مطالعه شدند. بیماران گروه مداخله، قبل از تعویض پانسمان علاوه بر دریافت مسکن در صورت لزوم، تحت 20 دقیقه تحریک کاتدی قشر حسی پیکری با شدت جریان یک میلیآمپر قرار گرفتند. در گروه کنترل پروتکل درمانی مذکور انجام شد با این تفاوت که پس از 30 ثانیه تحریک، دستگاه tDCS خاموش شد. مقیاس دیداری سنجش درد، در زمان قبل از انجام مداخله و پرسشنامه کوتاه و تعدیل شده کیفیت درد مک گیل پس از انجام پانسمان اجرا شد. دادهها توسط آزمونهای مجذور کای، تی مستقل و منویتنی بهوسیله نرمافزار SPSS نسخه 11.5 تحلیل شد.
یافتهها: دو گروه قبل از مداخله از نظر شدت درد زمینهای تفاوت آماری معنیداری نداشتند (p=0.08). در حین انجام پانسمان سوختگی دو گروه از نظر میانگین نمره بعد حسی، عاطفی و کل کیفیت درد تفاوت آماری معنیداری داشتند (p<0.05). علاوه بر این، اختلاف معنیداری بین شدت درد حین انجام پانسمان در گروه کنترل و مداخله وجود داشت (0.001>p)؛ اما نتایج تفاوت آماری معنیداری را بین بیماران دو گروه در رابطه با الگوی درد پانسمان سوختگی نشان نداد (0.14=p).
استنتاج: tDCS میتواند در کنار سایر روشها بهعنوان مداخلهای مؤثر و ایمن برای کاهش درد تعویض پانسمان سوختگی استفاده شود.

Background and purpose:The most experienced pain of burn injuries occur during dressing changes. This pain requires high doses of opioids. Some evidences suggest that changes in cortical irritability, cathodal somatosensory cortex stimulation, could be effective in relieving acute pain. The aim of this study was to determine the effect of Transcranial direct current stimulation (tDCS) on quality of pain in burn dressing.
Materials and methods: A randomized clinical trial including 60 eligible patients hospitalized in burn department of Imam Reza (AS) Hospital in Mashhad were divided into two groups through accessible sampling. In intervention group, patients received cathodal somatosensory cortex stimulation for 20 minutes with one mA current intensity before dressing change in addition to receiving painkillers (if necessary). The treatment protocol was performed in the control group too, however, the tDCS device was turned off after 30 seconds of stimulation. Pain visual analog scale was completed before the intervention and short and modified-form McGill pain questionnaire was applied after dressing. Data was analyzed by Chi-square, independent t-test and Mann- Whitney tests using SPSS software V. 11.
Results: There was no significant difference in background pain intensity before the intervention (P=0.088). Significant differences was found between the two groups in the mean scores of sensory, affective, and total pain quality during the burn dressing (P<0.05). Also, pain intensity of during dressing was significantly different between the control and intervention groups (P<0.001). However, in two groups the results showed no significant difference when comparing the pain pattern of burn dressing (P =0.145).
Conclusion: Transcranial direct current stimulation alongside other methods could be used as a safe and effective intervention for reducing the intensity of pain in burn dressing.
