fa نگرش و عملکرد پزشکان اطفال و زنان و زایمان شهرستانهای ساری و بابل در مورد قطع اقدامات احیا در نوزادان بسیار بدحال پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف:</span> تصمیم گیری در مورد انجام و یا عدم انجام عملیات احیا، مدت زمان ادامه و یا قطع آن در نوزادان بسیار بدحال با تردیدهای زیادی همراه است. این مطالعه به بررسی نگرش و عملکرد پزشکان اطفال و زنان و زایمان در این زمینه پرداخته است.<br><span style="font-weight: bold">مواد و روش ها:</span> در سال 1388، به کلیه پزشکان متخصص، فوق تخصص، دستیار و فلو اطفال و زنان و زایمان شهرهای ساری و بابل (140 نفر) یک پرسشنامه بی نام در این زمینه ارایه شد، و 75.7 درصد افراد به آن پاسخ دادند.<br><span style="font-weight: bold">یافته ها:</span> بیش از 70 درصد موارد، مخالف کمک به مرگ نوزاد بسیار بدحال، و کمتر از یک چهارم آنها موافق این کار بودند. هیچ کدام از پزشکان تاکنون دارویی برای تسریع مرگ این نوزادان تجویز نکرده بودند. قطع اقدامات احیا در موارد مختلف نوزادان بسیار بدحال شایع نبود و حداکثر به وسیله یک پنجم پزشکان به تشخیص خودشان در نوزادان دارای آنومالی های متعدد مادرزادی گزارش شد. عوامل تاثیرگذار در تصمیم گیری پزشک برای قطع اقدامات احیا در نوزادان بسیار بدحال به ترتیب عبارت بودند از تعداد فرزندان خانواده، فشار کارکنان بیمارستان، در خواست والدین نوزاد، جنس نوزاد، کمبود امکانات بیمارستانی، سابقه ناباروری والدین و وضع اقتصادی خانواده نوزاد. در بیشتر موارد اعتقادات مذهبی در تصمیم گیری برای ادامه و یا قطع درمان نوزاد بسیار بدحال موثر بود و منجر به موافقت بیشتر با تصویب قانونی در کشور برای شروع و یا ادامه درمان این نوزادان شده بود.<br><span style="font-weight: bold">استنتاج: </span>این مطالعه حاکی از غلبه نگرش تقدس زندگی بر نگرش کیفیت زندگی است. وضع قانون شفاف در این زمینه ضروری به نظر می رسد. </div> <span style="font-weight: bold">Background and purpose: </span>There are some debates with decision making for withdrawal/ withholding of life sustaining treatment in infants with adverse prognoses and incurable conditions. This study is conducted to review attitudes and practice of pediatrics and gynecology physicians in this field. <br><span style="font-weight: bold">Materials and methods:</span> This survey was conducted during 2009 in Sari and Babol. A total of 140 pediatricians, gynecologists and residents/fellows in these fields were recruited by census 106 doctors completed an anonymous questionnaire (response rate 75.7%). <br><span style="font-weight: bold">Results:</span> More than 70 percent of respondents disagreed and less than a quarter of them agreed with euthanasia. Thus far, none of the doctors prescribed drugs to expedite neonatal death. Withdrawal of life sustaining treatment in infants with adverse prognoses and incurable conditions was uncommon, and reported by one fifth of physicians in their diagnosis of infants with multiple congenital anomalies. Influential factors in their decision to withdraw life sustaining treatment in these infants were: the number of children, the insistence of hospital staff, the request from parents, the infant's sex, lack of hospital facilities, parental history of infertility and family income, respectively. Religious beliefs were effective for withdrawal/withholding of life sustaining treatment in the decision making process and was led to an agreement more with the approval of the law in the majority of cases. <br><span style="font-weight: bold">Conclusion:</span> This study revealed that attitude and sanctity of life, overcomes the attitude regarding to quality of life from most of the physicians.
