بررسی افسردگی اضطراب و کیفیت زندگی در بیماران با آمپوتاسیون اندام تحتانی در مقایسه با گروه کنترل در پی گیری 6 ساله

سابقه و هدف: آمپوتاسیون یکی از قدیمیترین روشهای جراحی است. هدف مطالعه حاضر بررسی افسردگی اضطراب و کیفیت زندگی در بیماران با آمپوتاسیون اندام تحتانی در مقایسه با گروه کنترل در پیگیری 6 ساله می باشد.
مواد و روشها: این تحقیق یک مطالعه توصیفی میباشد. با مراجعه به بایگانی بیمارستان امام خمینی (ره) ساری، لیست تمام بیمارانی که از سال82 تا 88 تحت آمپوتاسیون اندام تحتانی قرار گرفته بودند، استخراج و ضمن تماس تلفنی، آنان را به مصاحبه دعوت کرده تا پرسشنامه (SF12,HADS)را تکمیل نمایند. دادهها با استفاده از نرم افزار آماری SPSS و توسط آزمونهای توصیفی و تحلیلی T-test و Chi-square مورد تجزیه و تحلیل قرار گرفت.
یافتهها: شیوع افسردگی و اضطراب در 41 بیمار آمپوته نسبت به 41 فرد کنترل انتخاب شده متفاوت معنیدار نداشت (0= p، 77/0= p). در گروه بیماران میانگین متوسط نمره HADS افسردگی 8/498/9 و میانگین نمره اضطراب 6/48/9 بوده و در گروه کنترل این مقدار به ترتیب 22/449/9 و 58/434/10 بوده است. از نظر کیفیت زندگی در اجراء عملکرد فیزیکی، نقش فیزیکی و درد بدن بیماران آمپوته با گروه کنترل اختلاف معنیدار بود (05/0>p)، اما در سایر اجزاء شامل سلامت عمومی، نقش عاطفی، سر زندگی، سلامت ذهنی و فعالیت اجتماعی با یکدیگر مشابه و اختلاف معنیدار نبود (05/0p>).
استنتاج: در مطالعه ما ارتباط معنیداری میان شیوع علائم افسردگی و اضطراب پس از آمپوتاسیون در میان بیماران در مقایسه با گروه کنترل وجود نداشته است. همانطوری که از نظر کیفیت زندگی در اجزاء روانی اجتماعی مشابه بودند و از نظر اجزا جسمانی متفاوت بودند، بنابراین توجه به توانبخشی فیزیکی و رفع نواقص جسمانی مانند استفاده از پروتز اهمیت زیادی دارد.
Background and purpose: Amputation is one of the oldest surgical procedures. The aim of this study was to investigate the level of anxiety and quality of life in patients with lower limb amputation.
Materials and methods: In a descriptive study a list of all patients with lower limb amputation (between 2003-2009) was provided from the records in Sari Imam Khomeini Hospital. They were called and asked about their willingness to participate in the study. Data was then collected using the 12-Item Short Form Health Survey (SF-12) and Hospital Anxiety and Depression Scale (HADS). T-test and Chi-square test were applied in SPSS for data analysis.
Results: There were 41 patients and 41 individuals (control group) who were matched to patients based on demographic characteristics and underlying diseases. The mean HADS depression and anxiety scores in patients were 9.98±4.8 and 9.8±4.6 and in control group were 9.49±4.22 and 10.34±4.58, respectively. There were significant differences in mean quality of life subscales including physical functioning, physical role, and pain between patient and control group (P<0.05). But no significant differences were seen between the two groups in general health, emotional role, vitality, mental health and social activity (P>0.05).
Conclusion: In this study, no significant association was found in prevalence of depression and anxiety after amputation among patients and the control group. The two groups showed similar results in mental components but different outcomes in physical dimensions. Therefore, it is important to focus on physical rehabilitation programs and using prostheses in such patients.
