A comparison of single-layer versus multi-layer closure of the leg wound following long saphenous vein harvest for coronary artery bypass graft surgery: a prospective randomised controlled trial

<span style="font-weight: bold">سابقه و هدف:</span> بیماریهای قلبی- عروقی و مرگ ناشی از آنها متأسفانه طیف گستردهای را در علل مرگ و میر به خود اختصاص داده است. در دهههای اخیر جراحی قلب باز توانسته میزان مرگ و میر این دسته از بیماران را کاهش دهد. در این مطالعه به بررسی کاهش عوارض ناشی از بستن زخم پا در یک لایه و بررسی لزوم و یا عدم لزوم بستن لایه زیر جلدی زخم پا پس از برداشت ورید صافن بزرگ در اعمال جراحی بای پس کرونر پرداخته شد.<br><span style="font-weight: bold">مواد و روش ها:</span> این مطالعه به صورت کارآزمایی بالینی از نوع تصادفی انجام شد. جامعه مورد بررسی شامل 80 بیمار مبتلا به بیماری ایسکمی قلبی بودند که تحت عمل جراحی بای پس کرونر قرار گرفتند. در گروه اول زخم ناشی از برداشتن ورید صافن بزرگ تنها در یک لایه ترمیم شد و در گروه دوم زخم پا در چند لایه ترمیم شد. پس از اتمام مطالعه دادههای بدست آمده در مورد عوارض دو روش با استفاده از نرمافزار آماری SPSS17 تنظیم و دستهبندی شد و با کمک شاخصهای آمار مرکزی و آزمونهای t و chi-square مورد تجزیه و تحلیل آماری قرار گرفت.<br><span style="font-weight: bold">یافته ها: </span>بیماران دو گروه از لحاظ سن، وزن، قد، جنسیت و شاخص توده بدنی اختلاف معنی داری نداشتند. در روز دوم مطالعه تنها هماتوم تفاوت معنیداری (p=0.001) بین دو گروه داشت (در گروه یک لایه 5 بیمار و در گروه چندلایه 18 نفر). در روز چهاردهم مطالعه بجز در مورد نورالژی عصب صافن در 6 متغیر دیگر گروه چندلایه با عوارض بسیار بیشتری همراه بوده است. نتایج تقریبا مشابهی در روز بیست و یکم پس از عمل جراحی مشاهده شد.<br><span style="font-weight: bold">استنتاج:</span> شواهد بیانگر آن است که شیوه سوچور تک لایه زخم محل برداشت ورید صافن جهت بای پس عروق کرونری با عوارض و موربیدیته کمتری همراه میباشد. لذا توصیه میگردد در مواردی که از ورید صافن بزرگ به عنوان گرافت برای Revascularization عروق کرونر استفاده میشود برای جلوگیری از بروز اثرات نامطلوب نظیر عفونت زخم و یا درد مزمن و هماتوم از سوچور زخم به روش یک لایه استفاده شود.

<span style="font-weight: bold">Background and purpose:</span> Cardiovascular diseases is the leading cause of death in the world. Over the last decades, open heart surgery reduced the mortality rate of these patients. The long saphenous vein is most commonly-used conduit in coronary artery bypass surgery. Complications associated with its harvest can lead to significant patient morbidity. We aimed to determine the optimal method of wound closure in these patients by comparing single-layer with multi-layer wound closure techniques.
<br><span style="font-weight: bold">Materials and methods:</span> In a randomized controlled trial, eighty patients undergoing first-time elective coronary artery bypass were randomly divided into single-layer and multi-layer leg wound closure groups. In the first group, single-layer wound cloture technique, and in the second group, multiple-layer cloture technique was applied. Postoperative complications in both groups were analyzed using SPSS software (17) and descriptive statistical tests t-test and chi-square.
<br><span style="font-weight: bold">Results:</span> The two groups were comparable in respect of age, weight, height, sex and BMI. There was a significantly higher incidence of haematoma formation in the multi-layer closure group (n=18) compared with the single-layer closure group (n=5) (p<0.001). There was also a significantly higher incidence of saphenous nerve neuralgia in the multi-layer closure group on the 14th postoperative day. The incidence of postoperative pain and wound infection was also lower in the single-layer wound closure group.

<span style="font-weight: bold">Conclusion: </span>Single-layer leg wound closure following saphenous vein harvest for coronary artery bypass grafting is associated with fewer postoperative complications and does not impair postoperative wound-healing. We recommend its routine use in this clinical setting to prevent unwanted side effects such as wound infection and / or chronic pain and hematoma formation.
