Background and purpose: Hepatic hemangiomas are the most common benign liver tumors and are often asymptomatic. This study aimed to determine the epidemiology of hepatic hemangiomas and investigate factors associated with changes in tumor size in patients referred to gastroenterologists in Sari between 2017 and 2024.
Materials and methods: This descriptive study was conducted on 767 patients with a 7-year follow-up and four assessment visits. Demographic information, clinical symptoms, and changes in hemangioma size were collected and statistically analyzed during the follow-up.
Results: In this study, 54.4% of patients were over 40 years old, and 70.3% were female. Over 88% of patients had no associated symptoms at diagnosis. 95.2% of patients experienced a reduction in hemangioma size, 4.1% had an increase in size, and 0.7% showed no change. There were no significant differences in size changes (mm) based on gender (female 16.59 vs. male 18.26), medication history (none 16.85 vs. yes 18.60), or diabetes (none 16.82 vs. yes 20.91) (P>0.05). However, patients with a single hemangioma (18.13) had a greater reduction in size compared to those with two (14.11) or more (13.07) hemangiomas (P<0.05). Giant hemangiomas (48.32) showed significantly greater size reduction than smaller hemangiomas (15.23) (P<0.01).
Conclusion: This study revealed that hepatic hemangiomas are generally asymptomatic, and most patients experience a reduction in tumor size over time. Changes in hemangioma size were not related to gender, medication use, or diabetes, but giant hemangiomas showed significantly greater size reduction than smaller hemangiomas. These findings may inform clinical decision-making and patient management for patients with hepatic hemangiomas.
