Background and purpose: Ulcerative colitis (UC) is one of the most common types of inflammatory bowel disease, affecting the gastrointestinal tract and often presenting with extraintestinal manifestations. The oral cavity is commonly involved, and oral lesions may appear or worsen alongside the activity of the disease. Since oral health serves as an indicator of general health and a potential tool for the early detection of systemic conditions, understanding the pattern of oral manifestations in UC patients is clinically and preventively important.
Materials and methods: This cross-sectional descriptive-analytical study was conducted on 82 UC patients who visited Imam Khomeini Hospital in Sari in 2024. Patients were classified into active and inactive disease groups based on the Rachmilewitz Clinical Activity Index (CAI), and disease severity was determined according to Truelove and Witts criteria. Oral examinations were performed by a trained dentist, and lesions were recorded using the standardized Thomson–Murray questionnaire. Data were analyzed using the Chi-square test in SPSS version 22, with a significance level set at p<0.05.
Results: The mean age of participants was 44.99 ± 8.18 years. At least one oral lesion was observed in 57.3% of patients. Xerostomia (58.5%) was the most prevalent manifestation, showing a significant association with disease severity and activity (p<0.05). Other lesions included mucosal ulcers and erythema.
Conclusion: Oral lesions, particularly dry mouth, are common in UC patients. Regular assessment of oral manifestations should be considered part of patient monitoring and management, potentially aiding in the early detection of active disease periods.
