Background and purpose: Road traffic injuries (RTIs) remain a major public health concern, particularly during peak travel periods such as the Nowruz holidays in Iran. Mazandaran Province, one of the most visited regions during this period, experiences a marked increase in road traffic incidents. This study aimed to describe the epidemiology of RTIs during the Nowruz holidays (2021-2025) in Mazandaran Province and identify key predictors of pre-hospital mortality among cases attended by emergency medical services (EMS).
Materials and methods: We conducted a retrospective analysis of EMS records from Mazandaran Province covering five consecutive Nowruz holiday periods. Data were obtained from the Babol University of Medical Sciences, Mazandaran University of Medical Sciences, and other relevant organizations. Demographic, incident-related, and outcome data were analyzed to identify trends and risk factors associated with pre-hospital mortality.
Results: A total of 10,990 EMS missions were recorded, with a pre-hospital mortality rate of 2.9 deaths per 1,000 incidents. The majority of victims were young males (77.6%) with a mean age of 34 years. Age greater than 60 years, intercity location, and rural road accidents were identified as independent predictors of pre-hospital mortality. Despite increased traffic volume, the mean daily mission volume during the Nowruz period was lower than the annual average, possibly because traffic congestion reduced vehicle speeds, thereby decreasing the incidence of severe road traffic injuries.
Conclusion: This study highlights the elevated risk of pre-hospital mortality during the Nowruz holidays, particularly among older adults and individuals involved in crashes on high-risk roads. Targeted interventions focusing on speed management, road safety, and emergency preparedness during holiday periods may help reduce road traffic injuries and associated mortality in Mazandaran Province.
