Background and purpose: Obstructed Defecation Syndrome (ODS) is a common clinical problem. Stapled transanal rectal resection (STARR) is a relatively new technique in treating this disorder. The aim of this study was to evaluate the efficacy and complications of this technique in treating Iranian patients with ODS due to intussusception and/or rectocele

Materials and methods: A cross-sectional study was conducted in patients with ODS caused by intussusception and/or rectocele attending Colorectal Clinic in Tehran Rasool Akram Hospital, 2008-2010. Hundred and fifty patients with ODS were treated by medical treatment and biofeedback. In case of treatment failure, the patient was considered as candidate for surgery. Finally STARR procedure was performed in 61 patients with ODS. Clinical evaluation was done by ODS and incontinence scoring system.

Results: The mean age of the patients was 46.4±3.4 (range: 22-74 years) and the mean operation time was 38±9 minutes. Mean follow up time was 11.5±2.4 months. Mean ODS score improved from 14.7±0.9 preoperatively to 4.3±0.7 postoperatively (P=0.00). Mean pre and posoperative incontinence scores were 0.6±0.2 and 1.2±0.3, respectively (P=0.1). Among the subjects 84% were satisfied with operation and recommend the surgery to other patients.

Conclusion: STARR is a safe and effective method in treating ODS due to intussusception or rectocele.
