Patient safety is the key element of quality in healthcare. Ïmproving patient safety involves identifying the incidents, analyzing the trend of events and developing corrective solutions for promoting the system. Health care organizations can not judge the safety of the care without data and information related to patient safety. Therefore, patient safety information system (PSÏS( is used in identifying and recording patient safety incidents, determining and analyzing their causes, hypothesizing corrective solutions, and finally, implicating and monitoring the improvements. Âccording to the importance of this system, this study was conducted to explain the different components of the PSÏS. Ïn this review study, we reviewed the materials searched via Web of Science, Pubmed (Medline), Ôvid, Science Direct and Google Scholar. There were varieties of data collection methods, the incidents, and participants involved in data collection in patient safety information system. Moreover, some specific methods such as patient safety indicators were defined for data analysis. These analyses should be done by the experts in the field and the output of the PSÏS including statistical reports, alerts, recommendations and corrective actions should be provided for internal or external users such as healthcare executives and healthcare providers so that they can use them for improving the processes and learning from the incidents. Therefore, developing such a system in Ïran requires a framework for patient safety information system.
