Background and purpose: Coronavirus disease 2019 (COVID-19) emerged as a global public health crisis, resulting in substantial mortality, particularly among hospitalized patients. This study aimed to examine the epidemiological characteristics, factors associated with mortality, and the role of underlying diseases among patients with COVID-19 admitted to hospitals in Mazandaran Province, Iran, in 2020.
Materials and methods: In this cross-sectional study, data from 45,651 patients were extracted from the Health Care Monitoring Center system of Mazandaran University of Medical Sciences. Inclusion criteria consisted of a confirmed diagnosis based on a positive polymerase chain reaction test or compatible clinical symptoms and imaging findings. Statistical analyses were performed using the chi-square test, independent samples t-test, and both univariate and multivariate Cox proportional hazards regression models using SPSS software, version 25.
Results: The mean age of the patients was 58 years, with the highest proportion of cases (21%) occurring in the 60-69-year-old group. The overall mortality rate was approximately 8.8% and was significantly higher among male patients (9.5%) than female patients (8.2%). The highest mortality rates were observed in individuals aged over 80 years (19.7%), patients with renal failure (20.5%), those with cancer (......%?), and patients admitted to the intensive care unit (32.5%). Peripheral oxygen saturation below 93% was associated with a mortality rate of 19.5%, while intubation was associated with a mortality rate of 51.3%. Multivariate Cox regression analysis identified age greater than 50 years, male sex, underlying diseases (particularly renal failure, cancer, cardiac disease, diabetes, and chronic pulmonary disease), substance abuse, and hypoxemia as independent risk factors for mortality.
Conclusion: Advanced age, male sex, underlying diseases, and impaired respiratory function constitute the principal predictors of death among hospitalized COVID-19 patients. These findings can facilitate the early identification of high-risk groups, optimise clinical management, and support more effective allocation of healthcare resources during similar pandemics.
