The population status of Mazandaran province regarding diabetes-related indicators, self-care behaviors, and physical activity is concerning compared with other provinces; abdominal obesity and inactivity are more common among women and the urban population of the province than among men. In addition, inactivity is a significant health challenge for working women. This report presents policy options and recommendations aimed at improving physical activity among working women with diabetes in urban areas of Mazandaran province, in alignment with existing policy documents. The policy brief was developed using a systematic approach consistent with the WHO Integrated Knowledge Translation Model, including the stages of defining the purpose, engaging key stakeholders, identifying gaps and missing knowledge, gathering evidence, and presenting findings and recommendations. Triangulation of data was ensured by integrating scientific evidence, upstream policy documents, and the perspectives of key stakeholders and experts. The three proposed policy options include governance interventions, social interventions for community engagement, and individual-level actions. Implementing governance interventions is essential for enabling the simultaneous implementation of individual and social interventions, which can lead to the gradual improvement and refinement of current policies. Given women’s specific physical and psychological characteristics, time limitations, and dual responsibilities at home and in the workplace, ensuring the safety, effectiveness, and sustainability of individual actions is crucial. Furthermore, to operationalize intra-sectoral and intersectoral policy recommendations, it is advised that stakeholders adopt an integrated and provincial centralized approach through the Health and Food Security Working Group and the Family Physician Program, as these represent two key policy platforms.
