Background and purpose: Multiple myeloma is one of the most common hematologic malignancies, and its incidence has been increasing in many countries. Considering the impact of demographic changes and population aging, evaluating incidence trends at the provincial level is important for health system planning. This study aimed to investigate trends in the incidence of multiple myeloma in Mazandaran Province from 2015 to 2021.
Materials and methods: This retrospective cross-sectional study used population-based cancer registry data from Mazandaran Province. All newly diagnosed cases of multiple myeloma among residents of Mazandaran Province registered in the cancer registry of Mazandaran University of Medical Sciences between 2015 and 2021 were included using a census approach. Data collected included age, sex, year of diagnosis, county of residence, and place of residence (urban or rural). Data were analysed using SPSS software version 20. Descriptive statistics and inferential analyses, including the chi-square test, independent samples t-test, and one-way analysis of variance (ANOVA), were performed. A P-value < 0.05 was considered statistically significant.
Results: A total of 636 patients with multiple myeloma were identified in Mazandaran Province between 2015 and 2021. The mean age at diagnosis was 65.85 ± 12.70 years. The highest number of cases was recorded in 2021, whereas the lowest was observed in 2020. The age-standardized incidence rate (ASR) increased from 2.4 per 100,000 population in 2015 to 3.6 per 100,000 population in 2021, despite a temporary decline to 1.9 per 100,000 population in 2020. The mean age at diagnosis differed significantly across the study years (P< 0.05).
Conclusion: The findings indicate an overall increasing trend in the age-standardized incidence of multiple myeloma in Mazandaran Province during the study period. The temporary decline observed in 2020 was likely associated with disruptions in cancer diagnosis and registration during the COVID-19 pandemic. These findings are consistent with global epidemiological patterns and highlight the importance of continuous surveillance of multiple myeloma, strengthening population-based cancer registries, and prioritising the healthcare needs of an ageing population in health policy planning.
