Background and purpose: Considering the importance of high-quality midwifery care in reducing maternal and neonatal mortality, this qualitative study aimed to explore midwives' perspectives, preferences, and needs regarding improving the quality of care during labour. Although the quality of midwifery education in Iran is high, studies indicate some deficiencies in the quality of care provided. In addition, existing quantitative studies have not explored midwives’ experiences in depth.
Materials and methods: This study was conducted using a conventional content analysis approach based on 13 semi-structured interviews with midwives working at Shahid Rahimi Hospital in Khorramabad, who had between 4 and 20 years of work experience. The participants varied in age, marital status, and educational level. Through purposive sampling with maximum variation, data saturation was reached. Data were collected through in-depth interviews from December 2025 to May 2026. Interviews were audio-recorded, transcribed, and managed using MAXQDA software, version 22.
Results: Data analysis identified three main themes: (1) systemic and managerial challenges (including insufficient resources, unfavourable shift patterns, and lack of motivation), (2) education, communication, and collaboration (covering gaps in evidence-based practice, interaction with colleagues, and communication with patients), and (3) rights and dignity of stakeholders (addressing disrespectful care and the lack of recognition of professional views). Each theme encompassed multiple subthemes reflecting key barriers and needs in intrapartum midwifery care.
Conclusion: Improving the quality of midwifery care requires addressing systemic challenges, enhancing education, and protecting stakeholders' rights. The involvement of midwives in developing educational programmes and redefining professional job descriptions by health policymakers is essential. As primary providers of care, empowering midwives and addressing structural barriers can significantly improve maternal and neonatal health outcomes.
